The Coordinated Entry Case Conferencing: Adults & Families occurs monthly on the fourth Thursday at 2-3pm. The session is facilitated and the meeting invite is managed by Belle Homer (CE Lead) for the H2H CE program.
Topic of Meeting: The Adults & Families session is meant to have service providers review the list of participants on the H2H CE PL Report (who have requested assistance so referrals can be made for clients to connect with a provider, and any updates to client cases can be recorded. Program information and updates may be shared but is not the intended focus of this session.
Who should attend: Service provider staff who work directly with clients to provide case management, housing navigation, resource navigation, or other supportive services.
Providers who are currently represented include the HOTBHN Housing and Homeless Services team, The Salvation Army, the Mission Waco Meyer Center, Waco FAC, WISD HMSS, EOAC, HOCTILC, WHA, Throwing Aces Homeless Advocacy.
